About the Job
Objective/Overview:
As part of its integrated care model, Community-University Health
Care Center (CUHCC) has formed a Community Health Worker Team that
helps individuals to achieve their health goals and supports
population health at CUHCC and in the community. CHWs are fully
integrated members of patients' care health teams. Using their
cultural knowledge, language skills, life experiences and training,
CHWs support patients with health education, skill-building and
health care navigation through individual visits and group
classes/meetings. CHW will also conduct outreach activities to
promote health, assist with community-based care and/or serve as a
bridge back to the clinic. CHW work occurs primarily during regular
workday hours, but some evening and weekend hours may be available
or required. CHW will conduct their work in the clinic, and
patients' homes, and community settings (e.g., schools, public
housing, food shelves). There may be some flexibility for working
remotely. *Verbal fluency in Spanish or Hmong languages is required
for this position.
